About The Position

Anglicare’s Community Aged & Disability teams work across the Southern Queensland region, and are key in providing care and support to a range of clients, including older people and those with disabilities. They work with people to explore new options, open doors and create solutions that enable people to live well in their own homes. With the new Government led funding reforms, and to ensure we continue to offer the best possible support and care to our clients, Anglicare have introduced changes to our service model and are looking for a professional, capable and dedicated Multidisciplinary Team Coordinator in Woolloongabba.

This is a full time, permanent role and will be responsible for overseeing the coordination of clinical, nursing, allied health and specialist services to eligible clients; the leadership and management of the multidisciplinary team and the provision of operational support to the Service Manager, at the relevant Service Centre. Multidisciplinary Team Coordinators support the provision of human, material and educational resources across all services within the region, complying with funder accountability, quality accreditation and budgetary requirements.

 

About You

Offering a successful clinical career within the Community, Care or Health sectors - either in Nursing or an Allied Health field, you will be client-focused with exceptional leadership capabilities and your knowledge and background will include:

  • Applied knowledge and understanding of evaluation of clinical and therapy services and experience in daily operational activities of these services
  • Robust understanding of the ethical and legal requirements relevant to the programs, and provision of clinical governance
  • Change and progress management, operational implementation and oversight at a high level
  • Demonstrated skills in coordination, interaction and building effective, practical, respectful relationships within a multidisciplinary team
  • Previous management and leadership experience with ability to motivate and support others - remotely and across multiple sites
  • Capacity to support clients and their families through any issues, complaints or concerns and utilise skills to ensure mutually successful outcomes
  • Current practice and clinical skills to support staff and clients with hands-on service if required
  • Knowledge of continuous quality improvement and ability to provide organisational support through accreditation processes
  • Strong administrative and reporting skills; data and document control; computer programs including Microsoft suite

  • A focused and dedicated attitude to supporting staff, clients and families

Along with your passion and dedication to the sector, you will need:

  • Degree in Nursing or other Allied Health profession
  • Current registration with AHPRA and able to provide Statements of Service (as applicable)
  • Valid National Police Certificate
  • Queensland Driver Licence and your own, reliable vehicle
